ZERO Card ID

Hey, all you crazy zombie-slaying do-gooders.
Here's an ID template zombie license to kill for anyone to fill out. Use paint, photoshop, GIMP, Painter, whatever!


To Fill Out:

Name: What your parents gave your at birth or a nick name.
Age: Self explanatory.
Sex: Gender please.
Weight: How fat are you?
Height: Short or Gigantic?
Job: Are you a Tech Junkie, Sharp Shooter, Sniper, Smash 'N Bash, Chaser or Medic?
ID: For this, all you do is put the last two digits of the year you were born, the two digits of your day of birth, then the two digits of the month you were born in. Put a 1 in front of those if you're a boy, a 2 if you're a girl.
So, for me, it would be 2930601.
Carrier: This is for if you have been vaccinated for the Outbreak or not. More or less, you should be. So put Yes if vacinated or No if not. Each vaccination lasts at least 6 months, so put the expiration date of when you need a new shot.}
